Health Insurance - cost shared by Company and associate (will vary according to selected plan and number of covered dependents)

- No deductible for preventative care
- $50 individual / $150 family deductible

- Covers eye exam every 12 months

Effective After 90 Days
The following benefits are effective & available after 90-day probationary period
Paid Time Off (PTO) - cost paid by Company
- Accrues monthly from date of hire
- Number of PTO days varies by years of service:
- 1-5 years = 16 PTO days
- 10-14 years = 21 PTO days
- 20 years = 26 PTO days
- 30+ years = 31 PTO days
401(k) Plan - cost shared by Company & associate (will vary according to level of associate contribution)
- Company contributes 3% of associate’s annual salary
- Additional discretionary company contribution may be deposited
- Associate may contribute up to 75% of salary (subject to IRS maximum)
Incentive Plan - cost paid by the Company
- Pays a percentage -- up to 10% of associate's salary -- based upon the company's annual combined trade ratio (profit)
- Prorated for partial year employment
Life Insurance - cost paid by the Company
- Benefit is 2.5 times associate’s annual salary
- Associate selects beneficiaries at time of hire
Short Term Disability Insurance - cost paid by the Company
- First 5 days covered by PTO (or unpaid if no PTO)
- Days 6-30 company pays 100 percent of associate salary
- Pays 70% of associate salary for up to next 9 weeks
- Converts to long-term disability after 93 total days of disability
Long Term Disability Insurance - cost paid by the Company
- Begins after short-term disability has been exhausted
- Pays 60% of associate salary up to age 65
Effective After 1 Year
The following benefit is available after 1 year of service
Education Assistance - cost paid by the Company
- For college classes and other continuing education opportunities
- $5,250 annual maximum
